For Initial Visits Booked Online:
1. Download the Intake form by clicking on the link below "Patient Forms".
2. Download and save it to your desktop.
3. Open the form and type your name in to ensure you can save it.
4. Once you complete it please attach it to an email and send it back us ([email protected]). Also, if you are currently taking any supplements please bring them in with you to your appointment. Please note that the intake form is optional, however, the more information we have the better we can obtain a full case and treatment plan.
